VACORP Hybrid Disability Plan

If you are a VRS Hybrid Plan employee, you may be eligible to receive disability benefits through the VACORP Hybrid Disability Plan administered by Anthem Life. VRS Hybrid employees are automatically enrolled in the program which provides some income protection if you cannot work because of a physical illness, mental disorder, injury or pregnancy. Benefits include short-term and long-term disability coverage for an approved illness or disability condition. If you become disabled and are approved for short-term disability (STD) your income replacement is based on your years of service with Floyd County Public Schools (FCPS) and starts after 7 consecutive days of being disabled. Long-term disability (LTD) benefits are available after being disabled for 125 days.
*NOTE: FCPS opted out of the state disability plan and provides the VACORP Hybrid Disability Plan
administered by Anthem life as a comparable plan which meets all requirements of Title 51.1 of
the Code of Virginia.
Submitting a STD Claim

There are two methods to submit a Short-term Disability (STD) claim:

*To expedite the process, the Payroll/Benefits Office can initiate and online claim for employees. Please contact the Payroll/Benefits Office.
Upon initiation of a STD claim, Anthem Life will send the employee a disability information packet along with forms that are needed to complete the process. The required forms must be faxed, emailed, or mailed to Anthem Life at the following address:
